When dropping off forms for school, please make sure the following is complete:
Patient’s Full Name
Patient’s DOB
Specific Medications if required
Will you be picking it up or would you like it sent through MyChart
Please allow 5-7 business days for staff to complete Paperwork. We will call you when the form is ready to be picked up
